Peer Navigator

The Peer Navigator supports members experiencing or at risk of homelessness in the South Los Angeles and Compton areas by managing their health and motivating behavioral changes. This patient-facing role involves providing education, peer support, and resource navigation to improve health outcomes and facilitate housing placement. The position requires coordinating with medical and social services while maintaining accurate documentation in electronic health records.
