Practice Transformation Manager (Value-Based Care)

The Practice Transformation Manager (Value-Based Care) serves as a strategic advisor and primary liaison to providers, practice leaders, and internal stakeholders within the San Jose/Santa Clara County market. This role advances value-based care performance across the Accountable Care Organization (ACO) and affiliated Independent Practice Association (IPA) by evaluating provider performance and driving operational and clinical improvements. The position partners with regional and national teams to execute organizational priorities and enhance quality, utilization, and financial outcomes.
