Resource Navigator

The Resource Navigator guides youth aging out of foster care through a successful transition to independence by coordinating housing, education, employment, and care services. This role assesses youth needs, acts as a liaison between internal departments and community providers, and ensures seamless access to essential resources. The position involves managing intake, tracking referrals, and providing ongoing support to help vulnerable youth achieve self-sufficiency.
