RN Care Manager

The RN Care Manager provides comprehensive care coordination services to enrollees with high-complexity medical and behavioral needs enrolled in the Health Services for Children with Special Needs health plan. This role involves managing a caseload of medically fragile enrollees, assessing their needs, and developing individualized care plans in collaboration with multidisciplinary teams. The position requires conducting outreach, monitoring treatment progress, and facilitating transitions of care to ensure compliance and quality service delivery.
