RN Coordinator

The RN Coordinator facilitates team collaboration and care delivery within a comprehensive heart disease program, managing new patient contact, screening evaluations, and initial treatment communication. This role serves as a liaison with referring providers and hospitals to ensure seamless patient care across the continuum. The position also involves coordinating with clinical and administrative leaders to keep managers and providers informed of patient progression.
