RN Coordinator

The RN Coordinator role focuses on team collaboration and care delivery within a comprehensive heart disease program, managing new patient contact, screening evaluations, and initial treatment communication. This position serves as a liaison with referring providers and hospitals to ensure seamless patient care across the continuum. The coordinator works closely with clinical and administrative leaders to keep managers and providers informed of patient progression.
