Wraparound Facilitator

This role involves managing a caseload of families involved with DCFS and facilitating family team meetings to develop and implement comprehensive plans of care. The position requires conducting strength-based assessments, coordinating services, and advocating for families to meet their needs in the least restrictive settings possible.
