Case Manager

The Case Manager will screen, assess, and counsel homeless and formerly incarcerated participants with mental illness, substance use, or HIV/AIDS histories to link them with appropriate transitional and permanent housing. This role involves conducting intake interviews, creating individualized treatment plans, and providing ongoing counseling and supportive services to help clients achieve their housing and reentry goals. The position requires collaboration with internal departments and external referral agencies to ensure quality care and successful housing placements.
