Patient Navigator

The Patient Navigator identifies individuals and families in need of preventive and primary health care services, linking them to necessary resources such as acute, chronic, and communicable disease care, dental services, prenatal care, and pediatric immunizations. The role involves training and mentoring Patient Representatives, facilitating patient appointments, and assisting with referrals to specialty care and public health insurance programs. The position requires collaboration with medical staff to overcome barriers to care and ensure patients receive appropriate services.
