Wraparound Facilitator

The Wraparound Facilitator works with youth and families to identify strengths and needs for treatment and service planning. This role assists clients in making progress on their goals, resolving problems, and building skills and support networks. The position involves coordinating and individualizing services to meet the unique needs of a caseload of youth and families.
