MDS Coordinator

The MDS Coordinator is responsible for conducting and coordinating the development and completion of the Resident Assessment Instrument (RAI), Minimum Data Set (MDS), Care Are Assessments (CAAs), and Care Plans in compliance with State and Federal requirements. This role involves participating as part of the facility Interdisciplinary Team to manage patients receiving skilled services and performing other assigned nursing duties. The position requires coordinating assessments, monitoring documentation, and providing training to interdisciplinary team members.
