MDS - LPN

The MDS - LPN position involves coordinating the development and completion of the Resident Assessment Instrument (RAI), Minimum Data Set (MDS), Care Are Assessments (CAAs), and Care Plan in compliance with State and Federal requirements. The role requires participation in the facility Interdisciplinary Team (IDT) to manage patients receiving skilled services and perform other nursing duties as assigned. The position also includes training and mentoring staff on MDS item completion and nursing documentation.
