The MDS - LPN position involves coordinating the development and completion of the Resident Assessment Instrument (RAI), Minimum Data Set (MDS), Care Area Assessments (CAAs), and Care Plans in compliance with State and Federal requirements. The role requires participation in the facility's Interdisciplinary Team to manage patients receiving skilled services and ensure accurate documentation supports MDS coding. The position also includes providing training and mentoring to interdisciplinary team members and C.N.A. staff on MDS item completion and nursing documentation.
